These questions will be asked 24-48 hours prior to appointment and repeated at entry of patient for appointment. If necessary after screening some appointments may need to be rescheduled. We appreciate patience for the safety of all of us.
- Do you have a fever or above normal temperature?
- Have you been experiencing shortness of breath or have trouble breathing?
- Do you have a dry cough?
- Have you been experiencing GI issues or flu-like symptoms including diarrhea with associated muscle aches?
- Have you recently lost or had a reduction in your sense of smell?
- Do you have a sore throat not related to allergies?
- Have you or someone you have been in contact with tested positive for COVID-19 in the past 14 days?
- Are you or someone in your household awaiting COVID-19 testing results?
- Have you or a family member traveled via air, bus or train the past 14 days?
